Cedar Dental Group (“we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect when you visit cedardentalgroup.com (the “Site”) or contact us, how we use it, who we share it with, and the choices you have, including your choices about cookies and tracking technologies. By using the Site, you agree to the practices described here.

Information We Collect

Information you give us. When you fill out a form, call, email, or text us, we collect what you choose to share. This usually includes your name, phone number, email address, and appointment preferences and the reason for your visit (please keep health details out of web forms).

Information collected automatically. When you visit the Site, we and our service providers collect technical information through cookies and similar technologies. This includes your IP address, browser type and version, device type, operating system, referring website, pages viewed, time spent on pages, general location (such as city or region), and how you interact with the Site, such as clicks and form activity.

Information from third parties. Analytics and advertising providers we use may give us reports about how visitors use the Site and how our advertising performs. Those reports are generally summarized and do not identify you by name.

Cookies and Tracking Technologies

A cookie is a small text file stored on your device. Similar technologies, such as pixels, tags, and scripts, do the same kind of work. We use two kinds.

Essential technologies. These make the Site work, keep it secure, and remember your cookie choice. They do not require your consent.

Optional technologies (only with your consent). These help us understand how visitors use the Site and measure our advertising. When you first visit, a cookie banner asks you to Accept or Decline. Optional technologies are off until you click Accept. Before then, and if you click Decline, they do not load and no information is sent to them. If your browser sends a Global Privacy Control (GPC) signal, we treat it as a Decline and do not show the banner.

If you click Accept, the optional technologies on our Site are:

  • Analytics: Google Analytics (GA4) and Google Tag Manager, which report on Site traffic and usage.
  • Advertising and conversion measurement: Google Ads, which measure how our advertising performs and help show relevant ads.

Information collected by these tools may include your IP address, device and browser information, pages visited, actions taken on the Site, and a unique identifier. It is sent to the provider, which handles it under its own privacy policy: Google (https://policies.google.com/privacy).

Your choice and how to change it. We save your choice in a first-party cookie named c6cm_consent on your device for up to 365 days, so we do not ask on every page. You can change your choice at any time: Cookie Preferences. The same link appears in the footer of the Site. It reopens the banner. If you had accepted and then click Decline, the page reloads and the optional tools stop. You can also block or delete cookies in your browser settings, or use a GPC-enabled browser. If you clear your cookies, we will ask again on your next visit. Declining does not limit your ability to use the Site, call us, or request service.

Google Consent Mode. Our Site tells Google’s tools that analytics and advertising storage are denied until you accept, and updates that setting to match your choice.

Phone Calls and Call Tracking

We use a call tracking service (CallScaler) so we can tell which marketing source led you to call us. It may show a different tracking phone number on the Site depending on how you found us. When you call, the service may collect the phone number you call from, the date, time, and length of the call, and the marketing source of your visit. Because it is part of how the Site displays our phone number, call tracking is not switched off by the cookie banner.

Forms, Videos, Maps, and Other Third-Party Services

Forms. Our contact and request forms use the Gravity Forms plugin. What you submit is stored on our website and sent to us by email, and our hosting and email providers process it on our behalf.

reCAPTCHA. We use Google reCAPTCHA on forms to protect the Site from spam and abuse. reCAPTCHA collects hardware and software information, such as device and application data, and sends it to Google for analysis. Use of reCAPTCHA is subject to the Google Privacy Policy (https://policies.google.com/privacy) and Terms of Service (https://policies.google.com/terms). It is needed for security, so it is not switched off by the cookie banner.

Embedded videos and maps. Some pages include YouTube videos and Google Maps. When a page with one of these loads or plays, Google may set cookies and collect information about your visit, as if you had visited its website. See the Google Privacy Policy at https://policies.google.com/privacy.

How We Use Your Information

We use the information we collect to:

  • Respond to appointment requests and questions
  • Schedule and confirm visits, and send reminders
  • Communicate with you about your care and billing
  • Improve our website, services, and patient communication
  • Detect, prevent, and address fraud, spam, security, and technical issues
  • If you accept optional technologies: measure and improve our advertising and show relevant ads
  • Comply with legal obligations

How We Share Information

We do not sell your personal information for money. We share information only as needed to run our business and as described here:

  • Service providers that help us operate the Site and our business, such as website hosting, IT and security, email and text messaging, forms and scheduling, customer management, call tracking, analytics, and our marketing agency. They may use your information only to provide services to us.
  • Advertising and analytics providers, only if you click Accept on the cookie banner, as described above.
  • Legal and safety. We may disclose information when required by law, to protect our rights, property, or safety, or the safety of others.
  • Business changes. If our business is sold or reorganized, information may transfer to the new owner.

Sale and Sharing of Personal Information (Do Not Sell or Share)

We do not sell your personal information for money. Some state laws, including California’s, treat certain advertising activity as “sharing” personal information for cross-context behavioral advertising. If you click Accept, tools such as Google Ads may do this. If you click Decline, use Cookie Preferences to withdraw consent, or your browser sends a GPC signal, those tools do not run on the Site and we do not share your information in this way. We do not knowingly sell or share the personal information of anyone under 16.

Text Messaging (SMS)

If you give us your mobile number and agree to receive text messages, Cedar Dental Group may text you about appointments, service updates, and replies to your requests. Please do not send health information by text message. Consent to receive texts is not a condition of buying anything from us.

Message frequency and rates. Message frequency varies. Message and data rates may apply.

Opt out and help. Reply STOP at any time to stop receiving texts. Reply HELP for help, or contact us at 425-430-0400 or [email protected].

No sharing of mobile information. We do not sell, rent, or share your mobile phone number or your text message consent with third parties or affiliates for their marketing or promotional purposes. We may share it with service providers that help us send messages, only to deliver those messages for us.

Health Information and HIPAA

This Privacy Policy applies to information collected through this website, including cookies and similar technologies, contact forms, and phone calls made to the phone numbers on our Site.

If you are a patient of Cedar Dental Group, the health information we create or receive about your care is also protected by the Health Insurance Portability and Accountability Act (HIPAA). How we use and disclose that information, and your rights over it, are described in our Notice of Privacy Practices. If this Privacy Policy and our Notice of Privacy Practices differ for your health information, the Notice of Privacy Practices controls.

Please do not send sensitive health details, such as symptoms, diagnoses, or medical history, through the website contact forms, by text, or by email, because these channels may not be secure. To discuss your care, call our office at 425-430-0400.

How Long We Keep Information

We keep personal information only as long as needed for the purposes in this policy, to keep business records, and to meet legal requirements, and then delete or de-identify it. Analytics and advertising data is kept for the period set in each provider’s account, and we delete or de-identify it when it is no longer needed. Your cookie choice is kept for up to 365 days.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission over the Internet or electronic storage is completely secure, so we cannot guarantee absolute security.

Your Privacy Rights

Depending on where you live, including under the California Consumer Privacy Act as amended by the California Privacy Rights Act (CCPA/CPRA), you may have the right to:

  • Know what personal information we collect, use, disclose, and share, and why.
  • Access a copy of the personal information we hold about you.
  • Delete personal information we collected from you, subject to legal exceptions.
  • Correct inaccurate personal information.
  • Opt out of the sale or sharing of your personal information. Use the Cookie Preferences link or send a GPC signal, or contact us.
  • Limit the use of sensitive personal information. We do not use sensitive personal information to infer characteristics about you.
  • Not be treated differently for using these rights.

We will verify your identity by matching details you give us to information we already have, and we will respond within 45 days (we may extend once by another 45 days, and we will tell you if we do). You may use an authorized agent, and we may ask for proof of their authority. If we deny a request, you may ask us to reconsider.

Categories of personal information (past 12 months). We collected identifiers (name, email, phone number, IP address), internet or network activity (pages visited, time on site, referring URL, interactions with ads), approximate location from your IP address, commercial information (the services you ask about), and inferences drawn from the above for advertising, only if you accept optional technologies. We collect this from you directly and from your device. We disclose it for business purposes to the service providers described in this policy.

Shine the Light. California residents may ask us once a year which categories of personal information we shared with third parties for their direct marketing. We do not share personal information for third-party direct marketing.

Do Not Track. Some browsers send a “Do Not Track” signal. There is no common standard for it, so we do not respond to it. We do respond to Global Privacy Control signals as described above.

Children’s Privacy

Our Site is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has given us personal information, contact us and we will delete it.

The Site may link to websites we do not operate, such as social media pages. We are not responsible for their content or privacy practices. Please read their privacy policies.

Where Your Information Is Processed

We are based in the United States. Our service providers, including Google, may process information on servers in other locations. By using the Site, you understand your information may be transferred to and processed in those locations.

Changes to This Policy

We may update this Privacy Policy from time to time. The updated version will be posted on this page with a new effective date. If we make material changes to how we use optional tracking technologies, we will ask for your choice again.
